- Friday, May 20, 2016 at 9 a.m. to 2 p.m., Kenmore Community Center, 880 Kenmore Blvd., Akron, OH 44314
- Tuesday, May 24, 2016 at 9 a.m. to 12 p.m., Barberton Active Adult Center, 500 W. Hopocan Ave., Barberton, OH 44203
- Friday, June 10, 2016 at 10 a.m. to 2 p.m., Ellet Community Center, 2449 Wedgewood Dr., Akron, OH 44312
- Monday, June 20, 2016 at 10 a.m. to 2 p.m., Richfield Branch Library, 3761 Grant St., Richfield, OH 44286
To be eligible for the card, veterans must present a valid DD214 displaying an honourable discharge status and two forms of proper identification (one of which must contain a photograph): original or certified birth certificate; identification card issued by the United States Department of Veteran Affairs; United States military identification card; social security card; license or permit to carry a concealed weapon issued by this state or any other state; motor vehicle operator’s license issued by this state or any other state that bears a photograph of the licensee; identification card issued by the state or any other state that bears a photograph of individual identified; or valid passport that bears a photograph of the individual to whom the passport was issued. The fee for the card is $1.
