Revised DTE 100 and DTE 100 EX Conveyance Forms
The Conveyance Department in the Fiscal Office will be accepting only the State of Ohio Department of Tax Equalization (DTE) Forms, Conveyance Fee Statement DTE 100 and Exemption Fee DTE 100 EX for real property transfers.
The prescribed DTE Forms have the following date information in the upper left hand corner.
- Conveyance Fee Statement (Rev. 12/98) S.C. Date 01/06
- Exemption Form (Rev. 12/98) S.C. 01/06.
New DTE forms are available for pick up in our Property Transfer Department, located on the 4th floor of the Ohio Building in downtown Akron at 175 S. Main Street. You can also mail a request for the forms; include a stamped, self-addressed envelope with your request.

Sheriff’s Deed Legal Description Pre-Approval Checklist
- Completed Pre-Approval form
- A page from the foreclosure docket that reflects the Defendant(s) name and the case number
- A page from within the Complaint or Preliminary Judicial Report showing the address(s) of the property subject to foreclosure. Any vacant land subject to foreclosure must also be identified by street name
- The property legal description. The legal description must come from the property deed and must be typed on a separate sheet of paper. Each legal description must include the parcel number(s), routing number(s) and address(s) that it describes.
Summit County, Ohio Local Court Rules §11.01 through 12.05 regarding foreclosure filings have changed. For local court rules view Amendments to Rule 11 & 12 Foreclosures.
- All legal descriptions for Sheriff's deeds must be pre-approved by the Summit County Fiscal Office Property Transfer Department before filing the final judgment with the Summit County Clerk of Courts
- The legal descriptions are to be obtained from the property deed
